Registration Form Instructions

Step 1: Download the Registration form as Word Document or PDF and fill in information.  Please note that if you use the PDF with Adobe Acrobat Reader, the form can be filled out and printed, but it can not be saved and emailed. You can only save the data in the PDF form if you have the full Acrobat program. Or you can use the Word document save the filled out the form and email it. If you are not able to access the WORD form or save the PDF form, please print it and hand write the information onto the form and mail it (see step 3 below).

Step 2: Save the filled out form as DFL 2008 Registration and email to: DFL@DivingForLife.org  

Step 3: Mail check (U.S. funds) & copy of signed Registration Form (Page 2) to: Diving For Life, 241 W. 23rd Street #8 New York, NY 10011-2328 (Please use regular Mail; No Fed Ex or Airborne delivery services accepted)

You will receive an email confirming your application and check have been received (please allow one to two weeks for confirmation notice).
